To claim illness benefit from the Department of Social Protection (DEASP) you need to submit two things:
1) A Med1 cert, which we will electronically submit on your behalf AND
2) An IB1 which you need to fill in and send back to the DEASP.
Forms can be submitted to the DEASP in 2 ways:
1) Online: https://services.mywelfare.ie/en/topics/health-disability-illness/illness-benefit/ OR
2) Post a paper form to: FREEPOST, Social Welfare Services, PO Box 1650, Dublin
(The form can be obtained by either phoning 1890 800 024 or emailing forms@welfare.ie - Note that the DEASP have advised they cannot accept photocopied IB1 forms and you must use an original form).
Notes:
1) You must submit a new IB1 at the start of each new illness. You do not need to do it again if your cert is a continuation of a previous illness.
2) You will need to advise your employer to allow adjustment in payroll as required.
